PLAN BENEFITS

This plan offers four different benefits for covered accidents and provides cash benefits paid directly to you, or anyone you choose - benefits that are paid in addition to any other insurance you might have.

Hospitalization

Pays when a covered INJURY requires a hospital stay within 90 days of the INJURY. The benefit will also cover subsequent hospitalizations resulting from the INJURY for up to two years. The benefit is payable for a maximum of 365 days. Hospital does not include a nursing or convalescent home, skilled nursing facility, a place for treatment of mental illness, alcoholism or substance abuse, or a place for rest, custodial care, or care of the aged.

Living Benefits

Pays when a covered accident results in a serious injury, such as the loss of a limb.

Accidental Death

Pays when a covered person's INJURY results in death within 90 days from a covered accident listed below. Your benefits for Accidental Death will be paid: first, to your spouse; second, equally, to your children; third, to your estate.

Common Carrier

Pays double the Principle Sum when the insured is a fare paying passenger on a Common Carrier.

Automobile, Motor Home, Truck, or Pedestrian Accident
With this accident plan, you and your family (if insured) will be covered while driving or riding in an automobile, motor home, pickup truck, or as a pedestrian if struck by a motor vehicle on a public thoroughfare. Simple trips to the grocery store or to a friend's house can often lead to accidents that cause injury-sometimes very serious injury. With the Fraternal Order of Eagles Loyalty Advantage Accident Insurance Plan, you'll have insurance protection for those everyday, "around town" trips you make all the time.

Plane Bus Train Boat Taxi

Aircraft, Bus, Train, Ship, or Taxicab
Unlike some accident plans that only cover you when you're on an airplane, the Fraternal Order of Eagles Loyalty Advantage Accident Insurance Plan covers you on any public land or water conveyance operated by a Common Carrier, including as a passenger on a regularly scheduled commercial passenger airplane. This additional coverage helps protect you when you're on vacation, on a business trip, visiting the kids, or simply out enjoying yourself. With the Fraternal Order of Eagles Loyalty Advantage Accident Insurance Plan, you now have the assurance that your accident coverage is going with you wherever you go!

CASH BENEFITS

Your benefit amounts are determined by the number of continuous years you have been a Fraternal Order of Eagles member. With each member year, through the sixth year, your coverage will continue to grow with no premium rate increase.

YOUR EAGLES LOYALTY ADVANTAGE PLAN INSURANCE BENEFITS FOR ACCIDENTAL DEATH WILL BE PAID: FIRST, TO YOUR SPOUSE; SECOND, EQUALLY TO YOUR CHILDREN; THIRD, TO YOUR ESTATE.
This plan pays benefits in addition to any other insurance you have, including other Eagles plans.

ELIGIBILITY AND COVERAGE EFFECTIVE DATE
As an Eagles member, you are eligible to enroll regardless of your medical history if you are at least 18 years of age and a U.S. resident. Your coverage will take effect after your enrollment form and your first premium payment are processed by the Eagles Member Insurance Program office.

BENEFITS THAT PAY CASH DIRECTLY TO YOU
All benefit checks are made payable to you - and you can use the money however it will help you most. Plus, this plan pays benefits in addition to any other insurance you have, including other Eagles plans. Benefits are paid directly to you or to anyone you designate.

SPECIALLY-ARRANGED PREMIUMS
Your specially-arranged Eagles Loyalty Advantage Plan premium rates and/or benefits cannot be changed unless the same change applies to all members insured under the master policy.

YOUR PLAN CANNOT BE TERMINATED DUE TO AGE OR HEALTH
Your coverage cannot be cancelled by the insurance company as long as you remain a member of Fraternal Order of Eagles, pay the renewal premium when due, and the Plan's Master Policy remains in effect. Dependents' coverage ceases when your coverage terminates, premiums are not paid, or the dependents are no longer eligible.

NO-COST BENEFIT AMOUNT INCREASES
Your benefit amounts are determined by the number of consecutive years you have been a member of Fraternal Order of Eagles. With each member year your benefit amounts will continue to grow with no increase in premium. Maximum protection will accrue in the sixth year.

100% SATISFACTION GUARANTEE
You'll have a full 30 days to review your Certificate of Insurance. If the coverage is not everything you hoped it would be, just return your Certificate to us for a full refund of all the premium you have paid, minus any claims paid.

LIVING BENEFITS
100% of Principal paid for loss of: Life; Both Hands; or Both Feet; or Sight of Both Eyes; or One Hand and One Foot; or Speech and Hearing in Both Ears; or Either Hand or Foot and Sight of One Eye; or Movement of Both Upper and Lower Limbs (Quadriplegia). 75% of Principal paid for loss of: Movement of Both Lower Limbs (Paraplegia); or Movement of Three Limbs (Triplegia). 50% of Principal paid for loss of: Movement of the Upper And Lower Limbs of One Side of the Body (Hemiplegia); or Either Hand or Foot; or Sight of One Eye; or Speech or Hearing in Both Ears. 25% of Principal paid for loss of: Thumb and Index Finger of Either Hand; or Movement of One Limb (Uniplegia).

INCLUDE YOUR FAMILY
With Family coverage, your spouse, who is at least 18, a U.S. Resident and not legally separated or divorced from you, is covered for 50% of the amount of your coverage if your eligible children (unmarried children under age 19, older if full-time college student, see your Certificate of Insurance for more details) are covered; or 60% if you have no eligible dependent children covered. In addition, each child has 20% of the amount of your coverage if you have a spouse, or 40% if you have no spouse. Each eligible family member's protection increases each year based on your Eagles membership years, up to year six.

DEFINITION OF INJURY
Injury means bodily injury resulting directly from a covered accident and independent of all other causes.

DEFINITION OF COMMON CARRIER
Common Carrier means a conveyance operated by a concern, other than the policyholder, organized and licensed for the transportation of passengers for hire and operated by that concern.

EXCLUSIONS
The Policy does not cover any loss caused or contributed to by: 1) intentionally self-inflicted Injury; 2) suicide or attempted suicide, whether sane or insane; 3) war or act of war, whether declared or not; 4) Injury sustained while on full-time active duty as a member of the armed forces (land, water, air) of any country or international authority; (We will refund the pro rata portion of any premium paid for You or Your Dependents while You or Your Dependents are in the armed forces on full-time active duty, for a period of two months or more. Written notice must be given to Us within 12 months of the date You or Your Dependents enter the armed forces); 5) Injury sustained while On any aircraft except a Civil or Public Aircraft, or Military Transport Aircraft; 6) Injury sustained while On any aircraft: a) as a pilot, crewmember or student pilot; b) as a flight instructor or examiner; c) if it is owned, operated or leased by or on behalf of the Policyholder, or organization whose eligible persons are covered under The Policy; d) being used for tests, experimental purposes, stunt flying, racing or endurance tests; 7) Injury sustained while taking drugs, including but not limited to sedatives, narcotics, barbiturates, amphetamines, or hallucinogens unless as prescribed by or administered by a Physician; 8) Injury sustained while riding or driving in a scheduled race or testing any Motor Vehicle on tracks, speedways or proving grounds; 9) Injury sustained while committing or attempting to commit a felony. 10) Injury sustained while Intoxicated. Intoxicated means: 1) the blood alcohol content; 2) the results of other means of testing blood alcohol level; or 3) the results of other means of testing other substances; that meet or exceed the legal presumption of intoxication, or under the influence, under the law of the state where the accident occurred. Benefits will be paid for only one Type of Covered Accident under each Benefit payable under the Policy.
